Mechanism of Action of Menotrophin
Menotrophin is a mixture of hormones F.S.H.(follicle stimulating hormone) and L.H.( luteinizing hormone) which are natural anterior pituitary hormones which acts through cell membrane receptors and causing alteration of intracellular cyclic AMP concentration and alters protein kinase-A and thus regulates cell function.
FSH and LH acts to promote gametogenesis and secretion of gonadal hormones.
In female FSH induces follicular growth, development of ovum and secretion of hormone estrogen. In male FSH supports spermatogenesis and also has a trophic influence on seminiferous tubules. In female LH induces preovulatory swelling of the ripe Graafian follicle and triggers ovulation. It brings about luteinization of the ruptured follicle and maintains corpus luteum till the next menstrual cycle. Also only under luteinizing hormone`s influence progesterone secretion occurs. In males LH stimulates testosterone secretion by interstitial cells.

Contra-indications of Menotrophin
1. Hypersensitivity to the drug
2. Hormone dependent malignancies
3. Ovarian cysts
4.Men and women with tumours of the pituitary or hypothalamic glands in the brain
5.Men with prostate or testicular cancer
6.Women with breast, uterine or ovarian cancer
7.Women with vaginal bleeding of unknown cause
8.Women with ovarian cysts or enlarged ovaries, not due to polycystic ovary syndrome (PCOS)
9.Women whose ovaries are not functioning properly
10.Women with blocked fallopian tubes, unless having IVF.
11.Women with a physical defect of the reproductive organs (uterus, fallopian tubes, ovaries and/or cervix)
12.Women who have had a premature menopause or hysterectomy
13Women with uterine fibroids that may cause problems with a pregnancy
Special Precautions while taking Menotrophin
1. Stop treatment if precautious puberty occurs in patients of cryptorchidism
2. Endocrine disorders
3. Perform skin tests before therapy in susceptible individuals
4.Hyperprolactinemia or tumors of the pituitary or hypothalamus
5.Ovarian enlargement at risk of rupture, care in pelvic examinations
6.Risk of multiple births

Interactions for Menotrophin
1.Drugs with luteinising hormone activity may increase risk of ovarian hyperstimulation syndrome
2.Increased follicular response with clomiphene citrate
Typical Dosage for Menotrophin
Injection: Starts with 75 to 150 I.U. Adjust the dosage based on patient`s response. Dosage individualized. It depends on the condition and requirement of the patient. The correct dosage is to be determined by hormonal assay method. In schedule I; three equally divided doses on intermittent days and in schedule II; give daily doses until therapeutic response occur.

Store at a temperature range of 2 to 8 degree C.
Effects of Missed Dosage of Menotrophin
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.
Effects of Overdose of Menotrophin
Provide symptomatic treatment and supportive measures.
Symptoms: Ovarian hyperstimulation, ranging from mild to severe.
Management: In mild to moderate hyperstimulation: Bed rest, close observation and symptomatic relief. In severe hyperstimulation: Treatment is conservative and aims at restoring blood volume and preventing shock. If conception does not occur, acute symptoms will go away after several days and ovaries return to normal in 20 - 40 days. If conception occurs, symptoms may be prolonged.
