A synthetic sulfone(4,4`-diaminodiphenylsulfone (DDS)), Antileprotic,Antimalarial, Antibacterial.
Mechanism of Action of Dapsone
Dapsone is an antileprotic sulfone chemically related to sulfonamides & has same mechanism of action. It exerts it`s bacteriostatic action by acting as an antimetabolite by inhibiting bacterial folic acid synthesis. Para amino benzoic acid (PABA) is a constituent of bacterial folic acid. Dapsone acts as a structural analogue of PABA & thus inhibits enzyme bacterial folate synthetase. It competitively inhibits the union of PABA with pteridine residue to form dihydropteroic acid which conjugates with glutamic acid to produce dihydrofolic acid. Its antileprotic action in smaller doses is due to affinity of M.leprae to Dapsone

Absorption: Well absorbed orally, Distribution: Widely distributed & highly concentrated in skin, muscle, liver & kidney. Metabolism: It is acetylated as well as glucuronide and sulfate conjugated in liver. Excretion: Excreted primarily in urine & a small portion in urine & faeces.

Interactions for Dapsone
Rifampicin: Lowers dapsone levels even to ten fold by accelerating plasma clearance.
Folic acid antagonists: Increase the likelihood of hematologic reactions.
Para-aminobenzoic acid: Antagonises the effect of dapsone.
Activated charcoal: Decreases Gl absorption and enterohepatic recycling of dapsone.
Probenecid: Reduces urinary excretion of dapsone, increasing plasma concentration.
Typical Dosage for Dapsone
Adults: 50 to 100mg orally
Multibacillary leprosy: 100 mg daily with Rifampin& Clofazimine for12 months.
Paucibacillary leprosy: 100mg daily with Rifampin for 6 months
Dermatitis herpetiformis: 50mg/day increase up to 400mg for full control & then reduce dose
Acne: 100mg/day
Children: 50mg/day orally or 1 to 2mg/kg/day
Children: Adjusted dosage + Rifampin + Clofazimine for 12 months for multibacillary leprosy & adjusted dosage + Rifampin for 6 months for Paucibacillary leprosy

Storage Requirements for Dapsone
Store at room temperature below 30 degree C.
Effects of Missed Dosage of Dapsone
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.
Effects of Overdose of Dapsone
Treatment includes removal of drug from the body by Gastric lavage, Administration of activated charcoal & hemodialysis. Methylene blue is given for methaemoglobinaemia.
