Mechanism of Action of Ipratropium Bromide
Ipratropium bromide selectively acts on bronchial muscle without altering volume or consistency of respiratory secretions. It inhibits vagally mediated reflexes by antagonizing the actions of acetylcholine. It prevents the acetylcholine mediated increase in intracellular cyclic GMP concentration that results from interaction of acetylcholine with muscarinic receptor on bronchial smooth muscle. It does not depress the muco-ciliary clearance by bronchial epithelium. It blocks the cholinergic constrictor tone and acts primarily in the larger airways.

Typical Dosage for Ipratropium Bromide
Dosage individualized based on patients requirements.
Rhinorrhea, Allergic rhinitis, Perennial rhinitis:
Nasal spray (0.03%):
Adults and children over 12years: 2 sprays (42mcg) per nostril two to three times daily.
Nasal spray (0.06%):Rhinorrhea associated with common cold:
Adults and children over 12years: 2 sprays (84mcg) per nostril three to four times daily.
Children (5 to 12): 2 sprays (84mcg) per nostril three times daily

Chronic obstructive pulmonary diseases such as bronchitis, emphysema etc:
Inhalation aerosol:
Adults and children over 12 years: 2 to 4 inhalations or puffs every 8th to 6th hours. Increases if required up to 6 to 8 puffs thrice daily.
Inhalation solution:
Adults and children over 12 years: 250 to 500mcg in every 8th to 6th hours through a nebulizer

Asthma:
Inhalation aerosol:
Adults and children over 12 years: 1 to 4 inhalations or puffs every 6th hours as required. Children up to 12 years: 1 to 2 inhalations or puffs every 8th to 6th hours as required.
Children below 6 years: 1 inhalation; every 8th hours.
Inhalation solution:
Adults and children over 12 years: 500mcg every 8th to 6th hours through a nebulizer as required.
Children (5 to 12 hours): 125 to 250mcg through a nebulizer thrice daily. Increases up to every 4th to 6th hours if required

Storage Requirements for Ipratropium Bromide
Store at controlled room temperature at a range of 15 to 30 degree C. Protect from Direct heat, light, and moisture. Avoid freezing. Keep out of reach of children. Shake well before use.
Effects of Missed Dosage of Ipratropium Bromide
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.
Effects of Overdose of Ipratropium Bromide
