About Cyclosporin A
A cyclic nonribosomal peptide(11 amino acids) immunosuppressant drug, Organ transplant rejection prevention.
Mechanism of Action of Cyclosporin A
Cyclosporine binds to the cytosolic protein cyclophilin (immunophilin) of immunocompetent lymphocytes, especially T-lymphocytes. This complex inhibits calcineurin, which is responsible for activating the transcription of interleukin 2. It also inhibits lymphokine production and interleukin release and therefore leads to reduced function of effecter T-cells. Drug prevents mitochondrial PT pore from opening and inhibits cytochrome C release and also apoptosis induced by it.

Absorption- Absorption after oral administration varies from patient to patient. Rapidly absorbed after intravenous administration.
Distribution-Widely distributed in the blood. It crosses the placental barrier and enters to the breast milk.
Metabolism- Metabolized in the liver to its metabolites.
Excretion- Excreted through bile and urine (6%).

Adult-
Organ transplantation:10-15mg/kg/day, starting at 4-12 hrs before procedure and continued for 1-2 weeks, maintenance dose is 2-6mg/kg
Corneal transplantation.15 mg/ kg /day for 4-12hrs before transplantation. Continue this after surgery for 1-2 weeks Children:
Corneal transplantation.15 mg/ kg /day for 4-12hrs before transplantation. Continue this after surgery for 1-2 weeks
Intravenous-
Bone marrow transplantation: 3-5mg/kg/day for 2 weeks, maintenance dose is 12.5mg/kg
