Esomeprazole

About Esomeprazole
A proton pump inhibitor, Substituted Benzimidazole, Antiulcer, antisecretory drug.
Mechanism of Action of Esomeprazole
Esomeprazole is the S enantiomer of Omeprazole and is a proton pump inhibitor. It is a prodrug. After administration it diffuses in to the parietal cell of the stomach and accumulates in the secretory canaliculi. In the acidic medium Esomeprazole is converted to sulfenamide. This sulfenamide covalently interacts with sulfhydryl (SH) group in the proton pump (H+ K+ATPase) and inhibits the exchange of extracellular K+ for intracellular H+ ion. Esomeprazole irreversibly inhibits proton pumps activity and decreases gastric acid secretion. As compared to Omeprazole, Esomeprazole produces better control of intragastric acidity in Gastroesophageal reflux diseases. It is as effective as Omeprazole in other disease condition.
Pharmacokinets of Esomeprazole
Absorption: Esomeprazole is well absorbed after oral administration. Distribution: It is widely distributed in the body in protein bound form. Metabolism: Esomeprazole is extensively metabolised in the liver. Excretion: It is excreted mainly in the urine and small amount in faeces.

4.NSAID associated ulceration
5.Duodenal ulcer related to H. pylori infection
Interactions for Esomeprazole
Esomeprazole may potentially interfere with CYP2Cl9, the major esomeprazole metabolizing enzyme.
Esomeprazole may interfere with the absorption of drugs where gastric pH is an important determinant of bioavailability (e.g., ketoconazole, iron salts and digoxin). Esomeprazole does not have any clinically significant interactions with phenytoin, warfarin, quinidine, clarithromycin or amoxycillin.

Typical Dosage for Esomeprazole
Gastro-oesophageal Reflux Disease (GORD): Healing of erosive oesophagitis: 20mg or 40mg once daily for 4 to 8 weeks.
Maintenance of healing of erosive esophagitis: 20mg once daily.
Symptomatic gastroesophageal reflux disease: 20mg once daily for 4 weeks.
H. pylori eradication to reduce the risk of duodenal ulcer recurrence
Esomeprazole: 40mg, once daily for 10 days along with Amoxycillin and Clarithromycin

Schedule of Esomeprazole
H
Storage Requirements for Esomeprazole
Store at 15 - 30 degree C in a tightly closed container. Protect from light.
Effects of Missed Dosage of Esomeprazole
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.
Effects of Overdose of Esomeprazole
Give supportive measures and symptomatic treatment.

Domperidone

About Domperidone
Antidopaminergic, Motility stimulant,Piperidine derivative, Antiemetic, anti-vertigo.
Mechanism of Action of Domperidone
Domperidone is a potent dopamine receptor antagonist. It acts centrally and blocks the Dopamine receptor in the Chemoreceptor trigger zone and produces Antiemetic effect.
Domperidone acts peripherally in the gastrointestinal system and increases oesophageal peristalsis, oesophageal sphincter pressure and gastric motility. These all facilitates gastric emptying. Domperidone is used in migraine to relieve nausea and vomiting.
Anaesthetic adjuncts: It is used preoperatively in order to reduce the post operative vomiting.
Pharmacokinets of Domperidone
Absorption: Domperidone is rapidly absorbed after oral administration. Since it undergoes first pass metabolism oral bioavailability is only 15 %. Distribution: It is widely distributed in the body in protein bound form. Metabolism: Domperidone undergoes metabolism in the liver. Excretion: It is excreted mainly in the faeces and also in the urine.
Onset of Action for Domperidone
30 - 60 minutes
Duration of Action for Domperidone
6 - 8 hours
Half Life of Domperidone
7.5 hours
Side Effects of Domperidone
1.Diarrhoea
2.Dry mouth
3.Galactorrhoea
4.Gynaecomastia
5.Skin rash
6.Abdominal discomfort
7.Dystonic reactions
Contra-indications of Domperidone
1.Hypersensitivity to Domperidone
2.Prolactin releasing pituitary tumour(prolactinoma)
3.Gastrointestinal obstruction
Special Precautions while taking Domperidone
1.Hepatic impairment
2.Renal impairment
