Mechanism of Action of Fentanyl Citrate
This drug primarily acts on the CNS. Fentanyl is a pure Opioid agonist and primarily acts on the mu Opioid receptors located in brain, spinal cord, and in smooth muscles. This action causes alteration of pain perception in patients, thus providing analgesia. It has some hypnotic activity. It causes histamine release rarely.

Absorption-Rapidly absorbed after i.v, i.m, & transmucosal administration. Slowly absorbed after transdermal administration.
Distribution-Widely distributed in the body.80 to 85% bind to the plasma proteins.
Metabolism - Metabolize in the liver by N-dealkylation and hydroxylation. Nor Fentanyl is its active metabolite.
Excretion - Excreted via urine and faeces as unchanged & as metabolites.
Onset of Action for Fentanyl Citrate
Intravenous- 1 to 2min
Intrmuscular-7 to 15 min
Transdermal -12 to 24 hrs.
Transmucosal -5 to 15 min.
Duration of Action for Fentanyl Citrate
1/2 to 1 hr in the case of i.v
1 to 2 hr in the case of. i.m

Half Life of Fentanyl Citrate
Its plasma half life is 7 hrs in injections.
In transmucosal 5 to 15 min
In transdermal it is 18 hrs.

CNS depresants, neuroleptics, narcotic anlgesics, nitrous oxide, conduction anaesthetics.
Typical Dosage for Fentanyl Citrate
Adults-
Adjunct to general anesthesia-
2mcg/kg for minor procedures
2 to 20 mcg/kg i.v or i.m. for major procedures.
20 to 50 mcg/kg i.v. for complicated procedures.
Adjunct to anesthesia-
5mcg/kg lozenge.
Adjunct to regional anesthesia
50 to 100 mcg i.m or slow i.v over 1 to 2 min.
Post operative anesthesia
50 to 100mcg i.m every 1 to 2 hr.
Chronic pain
One transdermal system applies to a portion of the upper torso on an area of skin that isn`t irritated and hasn`t been irritated.
Children- 1.7 to 3.3 mcg/kg.

Store at 25 degree Celsius, preferably between 15 and 30 degree Celsius.
