Cholinergic agonist, urinary tract and GI tract stimulant.


Mechanism of Action of Bethanechol Chloride
It stimulates "Muscarinic cholinergic receptors" in the autonomic nervous system. This muscarinic acetylcholine receptor mediates various cellular responses, including inhibition of adenylate cyclase, breakdown of phosphoinositides and modulation of potassium channels through the action of G proteins. These receptors are found all over the body, including the muscle which surrounds the bladder and the muscles of the gastro-intestinal tract. Stimulation of the bladder muscle receptors enables the bladder to empty while stimulation of the esophageal (gullet) muscles prevents the reflux of stomach contents.

1. Blurred vision.
2. Vomiting.
3. Nausea.

Contra-indications of Bethanechol Chloride
1. Hypersensitivity to the drug or its ingredient.
2. Blockage of the gut (intestinal obstruction).
3. Obstruction of the urinary tract (urethra).
4. Recent heart attack
5. Recent surgical procedure.
6 Bradycardia.
7.Hypotension.
8. Epilepsy.
9.Parkinsonism.
10.Peptic ulcer.
11.Vasomotor instability.
Special Precautions while taking Bethanechol Chloride
1. Urinary retention, if the sphincter fails to relax as bethanechol contracts the bladder, urine may be forced up the ureter into the kidney pelvis
2. Bacteriuria, this may cause reflux infection.

Indications for Bethanechol Chloride
1. Acute postoperative and postpartum nonobstructive (functional) urinary retention.
2. Neurogenic atony of the urinary bladder with retention.
3. Heartburn
Interactions for Bethanechol Chloride
Ganglion blocking compounds may cause severe fall in B.P.
Cholinergic drugs: Additive effect may occur.
Quinidine, Procainamide: May antagonize cholinergic effects of bethanechol.

10-25mg 3-4 times daily either 1 hour before or 2 hours after meals

Store at room temperature; avoid freezing
Effects of Missed Dosage of Bethanechol Chloride
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.
Effects of Overdose of Bethanechol Chloride
Give symptomatic and supportive treatment. Administer atropine to reverse the adverse effects of drug. If needed epinephrine is administered to treat symptoms of bronchospasm.
