Selective Alpha1a Adrenergic Antagonist , For benign prostatic hyperplasia.
Mechanism of Action of Tamsulosin
Tamsulosin is adrenergic alpha receptor antagonist. It inhibits alpha 1A and alpha 1D subtype receptors. Tamsulosin selectively inhibits alpha 1A receptor in the prostate and leads to relaxation of smooth muscles in the bladder neck and prostate. This will improve urine flow and reduces BPH symptoms.
Adrenergic nerves releases adrenalin which produces contraction of muscles. Tamsulosin is selectively inhibit alpha 1A receptor in the prostate and bladder neck and relaxes the muscles and decrease the obstruction to the flow of urine in prostate hypertrophy.
Pharmacokinets of Tamsulosin
Absorption: It is well absorbed after oral administration under fasting condition.
Distribution: It is distributed widely to tissues in protein bound form.
Metabolism: It is extensively metabolised in the liver.
Excretion: The drug and its metabolites are excreted primarily in the urine and about 21% is excreted through faeces.

Special Precautions while taking Tamsulosin
1. During initiation of therapy slowly get up from chair or bed.
2. Avoid conditions in which injury could occur as a result of syncope.
3. Driving vehicle, operating machine and other hazardous woks.

Interactions for Tamsulosin
Cimetidine: Results in significant decrease in clearance of tamsulosin AUC.
Alpha adrenergic blocking agents: Tamsulosin should not be used in combination with other alpha adrenergics.
Typical Dosage for Tamsulosin
Adult: 400mg / day drug to be taken 30 minutes after the same meal each day and if sufficient response does not obtained then increase the dose to 800mg / day after 2 or 4 weeks.
Children: Not recommended

Effects of Missed Dosage of Tamsulosin
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose. If treatment with Tamsulosin is missed for several days, contact doctor before restarting the medication and a lower dose may be necessary to restart the drug.
Effects of Overdose of Tamsulosin
Give supportive measures and symptomatic treatment. Administer IV fluids and if needed give vasopressors. Monitor renal functions.
