About Finasteride
5 Alpha-Reductase Inhibitor, Synthetic anti-androgen.
Mechanism of Action of Finasteride
This synthetic 4-azasteroid derivative competitively inhibits steroid 5alpha reductase, which is an enzyme responsible for active androgen dehydrotestosterone (this enzyme influences the development of prostate gland, so decreasing level of this hormone relieve the symptoms associated with benign prostatic hyperplasia)from testosterone.
Pharmacokinets of Finasteride
Absorption- Poorly absorbed after oral administration
Distribution- Widely distributed in the body. It crosses the blood-brain barrier. 90% bound to the plasma proteins
Metabolism- Metabolized in the liver to its metabolites. Two metabolites, the t-butyl side chain monohydroxylated and monocarboxylic acid metabolites have been identified that possess not more than 20% of the 5alpha-reductase inhibitory activity of finasteride.
Excretion- It is excreted through urine and feces

Mechanism of Action of Tamsulosin
Tamsulosin is adrenergic alpha receptor antagonist. It inhibits alpha 1A and alpha 1D subtype receptors. Tamsulosin selectively inhibits alpha 1A receptor in the prostate and leads to relaxation of smooth muscles in the bladder neck and prostate. This will improve urine flow and reduces BPH symptoms.
Adrenergic nerves releases adrenalin which produces contraction of muscles. Tamsulosin is selectively inhibit alpha 1A receptor in the prostate and bladder neck and relaxes the muscles and decrease the obstruction to the flow of urine in prostate hypertrophy.

Absorption: It is well absorbed after oral administration under fasting condition.
Distribution: It is distributed widely to tissues in protein bound form.
Metabolism: It is extensively metabolised in the liver.
Excretion: The drug and its metabolites are excreted primarily in the urine and about 21% is excreted through faeces.

Adult: 400mg / day drug to be taken 30 minutes after the same meal each day and if sufficient response does not obtained then increase the dose to 800mg / day after 2 or 4 weeks.
