About Repaglinide
Meglitinide derivative, Insulin Secretagogue, Oral anti-diabetic drug.
Mechanism of Action of Repaglinide
It is a quick and short acting antidiabetic drug which exerts it`s action by stimulating insulin release from ?-cells of pancreas. It acts by binding to sulfonylurea receptor and to other distinct receptors and closes ATP dependent potassium channels and thus causes depolarization. This depolarization of pancreatic ? cells causes opening of Ca channels and increased Ca influx which induces insulin secretion.

Absorption: Well absorbed orally, Distribution: Widely distributed in a highly protein bound form, Metabolism: Metabolized in to inactive metabolites, Excretion: About 90% of the drug is excreted through faeces as metabolites and rest is excreted through urine mainly as metabolites and less than 0.1% as parent drug.

Contra-indications of Repaglinide
1.Hypersensitivity to the drug
2.Type-1 diabetes mellitus
3.Diabetic ketoacidosis
4.Severe hepatic impairment
Special Precautions while taking Repaglinide
1.Hepatic impairment
2.Malnourished and deliberate individuals
3.After severe exercise
4.Along with alcohol
5.Along with other glucose lowering drugs
6.In patients whose HbA1c is below 8%
7.Loss of control of blood glucose may occur with drug when patient is exposed to stress such as fever, trauma, infection, or surgery. In such situations stop the Repaglinide and administer insulin

Indications for Repaglinide
Non-insulin dependent diabetes mellitus (as adjunct to diet and exercise)
Interactions for Repaglinide
Ketoconazole, miconazole and erythromycin: Metabolism of repaglinide inhibited.
Troglitazone, rifampicin, barbiturates and carbamazepine: Increase metabolism of repaglinide.
Typical Dosage for Repaglinide
Oral: For patients not previously treated or whose HbA1c is below 8%; starts with 0.5mg 30minutes before meals. For patient`s previously treated with glucose lowering drugs and whose HbA1c is 8% or more starts with 1 to 2mg immediately 30minutes before each meals. Take 3 to 4 times a day based on changes in the patient`s meal pattern. Adjust dosage based on blood glucose response at weekly intervals.
Maintenance dose: 0.25 to 4mg prior to meals.
Maximum dose: 16mg/day.

Store at room temperature below 25 degree C.in a well closed container. Protect from moisture. Keep out of reach of children.
Effects of Missed Dosage of Repaglinide
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.
Effects of Overdose of Repaglinide
Provide symptomatic treatment and supportive measures. Treat mild hypoglycaemia with glucose and dosage or meal pattern adjustment. Closely monitor patient for at least 24 to 48 hours. Treat severe hypoglycaemia with neurological manifestations with I.V. dextrose 50% solution followed by a continuous infusion of glucose 10%solution. Carefully monitor blood sugar levels.
