About Clomiphene
A selective estrogen receptor modulator (SERM),chlorotrianisene derivative, Ovulation Stimulator ,Fertility agent.
Mechanism of Action of Clomiphene
The drug is an estrogen antagonist which exerts it`s pharmacological action by inducing ovulation. It induces gonadotropin secretion in women by blocking estrogenic feedback inhibition of pituitary. It increases the amount of LH and FSH released at each secretary pulse which results in enlargement of ovaries and ovulation.
In men increased gonadotropin levels results in increased steroidogenesis and helps in oligospermia and infertility.

Absorption: Well absorbed orally. Distribution: Distributed preferentially in to the adipose tissue. Metabolism: Metabolized in liver. Excretion: Excreted mainly through bile and faeces.

7. Pituitary tumor.

Indications for Clomiphene
1. Anovulatory infertility
2. Oligomenorrhoea
3. Post pill amenorrhoea
4. Oligospermia
5. Polycystic ovary syndrome
6. Lactation amenorrhoea.
7. Psychogenic amenorrhoea
8. Menorrhagia due to hyper plastic endometrium.
Interactions for Clomiphene
Gonadotropin Therapy: In combination, Gonadotropin therapy causes hyper-stimulation of ovary.
Lab Tests: Levels of Thyroxine and Thyroxine-binding Globulin (TBG) increased.
BSP (Bromsulphalein) Lab studies: greater than 5% retention of BSP reported in 10-20% of patients.
Typical Dosage for Clomiphene
Oral:
Anovulatory infertility: 50mg once daily for 5days starting from 5th day of menstrual cycle. (Starting from any time in women who have had no recent uterine bleeding). Dose may be increased to 100mg if ovulation does not occur. Repeat the 5 day course until conception occurs or until the completion of 3 courses.
Male infertility: 25mg daily for 25 days followed by a rest period of 5days for 3 to 6 months.

Storage Requirements for Clomiphene
Store at controlled room temperature at a range of 15 to 30 degree C.in a well closed container. Protect from light, excess heat and moisture.
Effects of Missed Dosage of Clomiphene
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.

Provide symptomatic treatment and supportive measures.
