About Medroxyprogesterone
Progestogenic hormone, Hormonal contraceptive.
Mechanism of Action of Medroxyprogesterone
It is a progestin derivative exerts it`s action through nuclear progesterone receptors situated in female genital tract, breast, CNS, and pituitary. When the drug binds to progesterone receptors it undergoes dimerization and attaches to progesterone response element of target genes and regulates transcription through coactivators; which ultimately leads to modification of protein synthesis. It contributes to proper development and maintenance of female sexual functions. It prepare uterus for nidation and maintenance of pregnancy. It prevents endometrial shedding, decreases uterine motility, and also inhibits immunological rejection of the foetus by decreasing T-cell function and cell mediated immunity.

Absorption: Well absorbed orally. Slowly absorbed after I.M. administration.
Metabolism: Metabolized in liver in to metabolites and undergoes extensive first pass metabolism.
Excretion: Excreted mainly through urine and also excreted through faeces.

Interactions for Medroxyprogesterone
Cyclosporin: Increased plasma cyclosporin concentrations.
Rifamycins: Metabolism accelerated by rifamycins.
Typical Dosage for Medroxyprogesterone
Oral:
Endometriosis: 10mg 8th hourly from the first day of the menstrual cycle to 90 consecutive days.
Dysfunctional uterine bleeding: 2.5mg to 10mg/day for 5 to 10days; preferably on 16th to 21st day of menstrual cycle. Repeat on 2 to 3 subsequent cycles.
Secondary amenorrhea: 2.5 to 10mg for 5 to10 days, beginning on 16th to 21st day of menstrual cycle. Repeat on 3 successive cycles.
Progesterone component in replacement therapy for menopausal symptoms: 5 to 10mg/day for 12 to 14 days.
Palliative treatment of endometrial, renal, and prostatic carcinoma: 100 to 500mg/day.
Contraception:
I.M.: 150mg intramuscularly provide contraception for 3 months.

Effects of Missed Dosage of Medroxyprogesterone
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.
