About Corticotrophin
Anterior pituitary hormone, a polypeptide hormone, Diagnostic aid,hormone replacement therapy,in multiple sclerosis.
Mechanism of Action of Corticotrophin
It is an anterior pituitary hormone which promotes steroidogenesis in adrenal cortex. It acts through specific G protein coupled receptors by stimulating cyclic AMP formation in cortical cells and rapidly increases the availability of cholesterol for conversion to pregnenolone which is the rate limiting step in the production of glucocorticoids, mineralocorticoids and weakly androgenic steroids. It also has a trophic influence on adrenal cortex which also mediated through cyclic AMP.

Typical Dosage for Corticotrophin
Adult:
I.V.:
Diagnostic test of adrenocortical function: 10 to 25 U in 500ml of 5% dextrose injection I.V. over 8 hours between blood samplings.
I.M. or S.C.
Hormone replacement: 20U 6th hourly.
Allergic reactions, Collagen disorders, Dermatologic disorders, Inflammations: 40 to 80U/day. Adjust dosage based on patient`s response.
I.M.:
Exacerbation of multiple sclerosis: 80 to 120mg/day for 2 to 3 weeks
Infantile spasm: 20 to 40U daily or 80U every other day for 3months or 1month after spasm ceases.

Store at a temperature range of 2 to 8 degree C.
Effects of Missed Dosage of Corticotrophin
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.

Provide symptomatic treatment and supportive measures.
