Pituitary hormone, Growth inhibitor.
Mechanism of Action of Somatostatin
The drug exerts it`s action by inhibiting the secretion of Growth hormone (GH), Thyroid stimulating hormone (TSH), Prolactin in pituitary; Insulin and glucagons in pancreas, and all gastrointestinal secretions including gastrin and HCl. It constricts splanchnic, hepatic, and renal blood vessels. It decreases blood flow to gastrointestinal mucosa and thus controls bleeding esophageal varices and bleeding peptic ulcer. It controls pancreatic, intestinal, or biliary fistulae due to antisecretory effect.

Indications for Somatostatin
1. Severe acute bleeding from intestinal varices, Gastric or duodenal ulcers, or haemorrhagic gastritis
2. Pancreatic, Intestinal and biliary fistulae
3. Prophylactic treatment of pancreatic surgery
4. Diabetic ketoacidosis
5. Hormone secreting tumors such as Insulinomas, Glucagonomas, VIPomas, Carcinoid tumors, Growth hormone secreting adenomas
6. Acromegaly
Interactions for Somatostatin
Typical Dosage for Somatostatin
I.V.: 250mcg as slow I.V. bolus injection over 3 to 5minutes followed by continuous I.V. infusion at a rate of 3mg/12hour.
Severe acute bleeding from intestinal varices, Gastric or duodenal ulcers, or haemorrhagic gastritis: Initially 250mcg slow I.V. injection followed by slow IV. Infusion at a rate of 250mcg/hour. If haemorrhage stops continue treatment for 2 to 3days.
Pancreatic, Intestinal and biliary fistulae: Continuous I.V. infusion of 250mcg/hr until closure of fistulae. Continue treatment for 24 to 72 hours more.
Prophylactic treatment of pancreatic surgery: Continuous I.V. infusion of 250mcg/hr at the beginning of surgery and continue treatment for 5days.
Diabetic ketoacidosis: Continuous I.V. infusion at a rate of 100 to 500mcg/hour along with insulin.

Storage Requirements for Somatostatin
Store at a temperature range of 2 to 8 degree C.
Effects of Missed Dosage of Somatostatin
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.

Effects of Overdose of Somatostatin
Provide symptomatic treatment and supportive measures.
