Mechanism of Action of Diclofenac (Eye Prep.)
Diclofenac possess analgesic, anti-inflammatory and antipyretic action. It inhibits the enzyme cyclo-oxygenase and there by inhibits the synthesis of Prostaglandins (((PGs))). It is more potent against cyclo-oxygenase-2 enzyme as compared to other NSAIDs like Indomethacin, Naproxen etc. It reduces intracellular concentrations of free arachidonic acid in leukocyte by altering its release or uptake

Typical Dosage for Diclofenac (Eye Prep.)
Diclofenac sodium 0.1%w/v is used. 1-2 drops is instilled in to the eye every 6 - 8 hours a day.
Ocular surgery: Starts from 3 hours before surgery as 1 drop every hour followed by 1 drop 3 times daily after surgery.
Treatment of post operative ocular inflammation: 24 hours after surgery instill 1-2 drops for 4 times daily.
Prevention of surgically induced miosis: Instill 1 - 2 drops 4 times within 2 hours before surgery.
Post-photorefractive keratectomy pain: Instill 1 - 2 drop in to the eye 2 times within 1 hour before surgery followed by 1 drop twice at 5 minutes interval after surgery and 1 drop every 2 - 5 hours after1 day.
Cataract Procedures: Pre-operatively: instill 1 drop in the conjunctival sac up to 5 times within 3 hours before surgery. Post-operatively: instill 1 drop in the conjunctival sac 15, 30 and 45 minutes following surgery, then 3 to 5 times daily, for as long as required.
Inflammatory Conditions (((conjunctivitis, keratoconjunctivitis, and corneal ulcers))): instill 1 drop in the conjunctival sac 4 to 5 times daily, depending upon the severity of the disease.
