About Piroxicam
NSAID,Oxicam derivative, Analgesic,antiinflammatory.
Mechanism of Action of Piroxicam
Drug inhibits the biosynthesis and release of prostaglandins by inhibiting cyclooxygenase enzyme in a reversible manner. In this way it reduces the PG (Prostaglandin) concentration in the synovial fluid. It also reduces platelet aggregation (prolong bleeding time), production of IgM rheumatoid factor, chemotaxis of leukocytes and ratio of T-helper to T- suppressor lymphocytes.
Pharmacokinets of Piroxicam
Absorption- Rapidly and completely absorbed after oral administration.
Distribution- Widely distributed in the body as plasma protein bound form (99% bound to the plasma proteins). The drug is distributed into breast milk.
Metabolism- Metabolized in the liver by hydroxylation and glucoronide conjugation to its metabolites.
Excretion- Excreted through urine and bile.
Onset of Action for Piroxicam
Analgesic action: 1 hour after administration
Antirheumatic action: 7- 12 days after administration

Indications for Piroxicam
1. Rheumatoid arthritis
2. Osteoarthritis
3. Ankylosing spondylitis
4. Acute musculo-skeletal disorders
5. Acute gout
6. Post-operative & post-traumatic pain
7. Primary dysmenorrhea
Interactions for Piroxicam
Anticoagulants: May prolong prothrombin time.
Beta-Blockers: Antihypertensive effect may be impaired.
Cyclosporine: Nephrotoxicity of both agents may be increased.
Lithium: Serum lithium levels may be increased.
Hydantoins: Serum hydantoin levels increased resulting in increased pharmacologic and toxic effects.
Methotrexate: Risk of methotrexate toxicity (stomatitis, bone marrow suppression, nephrotoxicity) increased.
Probenecid: May increase the concentration and toxicity of piroxicam.
Aspirin: Displaces piroxicam from serum binding sites resulting in increased incidence of G.I. effects.
Typical Dosage for Piroxicam
Oral-
Adults-
General dose - 20mg daily as a single dose
Musculo-skeletal disorders- Initially 40mg daily for 2 days then 20mg daily for 7-14 days.
Gout- Initially 40mg as a single then 40mg daily in single or divided doses for next 4-6 days.
Post-operative & post-traumatic pain- 40mg daily in single or divided doses for first two days, then 20mg once daily.
Primary dysmenorrhea- 40mg once daily for first two days, then 20mg once daily for 2-3 days.
Injection-
Same as oral dose by deep I.M. injection into upper outer quadrant of buttocks. For maintenance use oral preparations
For Topical application:
Adults-
Apply 1g gel to affected site 3-4 times daily. Review therapy after 4 weeks.

Storage Requirements for Piroxicam
Store it at room temperature and away from excess heat and moisture. Keep out of reach of children.
Effects of Missed Dosage of Piroxicam
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.
Effects of Overdose of Piroxicam
Give symptomatic and supportive treatment. Induce emesis by Ipecac or gastric lavage. Administer activated charcoal through nasogastric tube. Monitor vital signs and laboratory parameters regularly.
