Mechanism of Action of Procarbazine
This drug inhibits the incorporation of small DNA precursors, as well as RNA and protein synthesis. It also causes DNA damage through an alkylation process. During alkylation the drug first converted to azoprocarbazine by oxidation. N-oxidation of this generates benzylazoxy and methylazoxy derivatives. This methylazoxy compound generates a potent methylating reagent which involved in cytotoxicity.

Absorption- Rapidly and completely absorbed after oral administration.
Distribution - Widely distributed in the body. It crosses the blood brain barrier.
Metabolism - Metabolized in the liver to its metabolites. Azoprocarbazine is its active metabolite.
Excretion - Excreted through urine (as N-isopropyl-terphthalamic acid).

Interactions for Procarbazine
Vaccine Live: Use of live vaccine to be avoided with procarbazine.
Alcohol : Disulfiram-like reaction.
Phenytoin: reduced absorption of phenytoin.
Typical Dosage for Procarbazine
Oral-
Adult-
2- 4mg/kg/day in single or divided doses for the first week, followed by 4-6 mg/kg/day.
1-2 mg/kg/day as maintenance dosage.
Children-
50mg/m2/day for first week, followed by100mg/m2/day.
50mg/m2/day as maintenance dose.

Storage Requirements for Procarbazine
Stored at room temperature. Protect from light. Keep out of reach of children.
Effects of Missed Dosage of Procarbazine
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.
Effects of Overdose of Procarbazine
Give supportive and symptomatic treatment. Supportive treatment includes blood component transfusion, antiemetics, antianxiety agents and antipyretics.
