Mechanism of Action of Ifosfamide
In the liver drug undergo enzymatic activation into 4-hydroxyifosfamide.This alkylates the DNA by binding to N7 position of the guanine residue. This results cross linking/abnormal base pairing/scission of DNA strand. There is cross linking of nucleic acid with proteins can also takes place. The overall process is cell cycle nonspecific reaction.

Mechanism of Action of Mesna
Mesna rapidly oxidized to major metabolite, mesna disulfide (dimesna). In the kidney dimesna reduced into free thiol compound. This compound reacts with the metabolites of oxazaphosphorins (cyclophosphamide or ifosfamide) to form stable nonurotoxic compounds. During this detoxification there is binding of mesna to the 4-hydroxy-ifosfamide forming a nonurotoxic 4-sulfoethylthioifosfamide.Mesna also bind to the double bonds of acrolein and to other urotoxic metabolites.

Typical Dosage for Mesna
Intravenous-
Adults-Mesna dose depend upon the amount of ifosfamide and cyclophosphamide that a patient receives. The standard dose depends equal to 20% of the total ifosfamide given at three separate time interval through a vein infused over at least 5 minute, first dose is right before the ifosfamide, often referred to as 0 hr, the 2nd dose is 4 hr after the start of the infusion and the 3rd dose is 8 hrs after the start of infusion.
Mesna can also be administered through orally by mixing with flavoured drink like grape juice, cola, chocolate and milk etc.
