Mechanism of Action of Dimenhydrinate
Antiemetic and antivertigo action: Dimenhydrinate is a salt of Diphenhydramine and Chlorotheophyllinate. Dimenhydrinate produces Antiemetic action by its central action on Chemoreceptor trigger zone (CTZ). It decreases the sensitivity of Labyrinth apparatus which transmit stimuli to the CTZ and stimulates the vomiting center.
Antiallergics: Dimenhydrinate is a potent H1 receptor antihistaminic agent. It has anticholinergic and central sedative action also.
Migraine: Dimenhydrinate is used in migraine due to its sedative as well as antiemetic actions.

Absorption: Dimenhydrinate is well absorbed after oral administration. Distribution: It is distributed widely in the body and crosses the placenta. Metabolism: Dimenhydrinate undergoes metabolism in the liver. Excretion: Excreted mainly in urine.

Indications for Dimenhydrinate
1. Treatment of nausea, vomiting and dizziness associated with motion sickness.
2. Treatment of nausea and vomiting in Meniere`s disease
Interactions for Dimenhydrinate
Alchohol & other CNS depressants like barbiturates, analgesics, sedatives neuroleptics: CNS depressant effect enhanced.
MAOIs, Atropine & TCAs: Antimuscarinic effect enhanced.
Aminoglycosides: Ototoxicity masked, irreversible damage may occur.
Aminophylline, Hydrocortisone, Phenothiazines and some soluble barbiturates: Incompatible with these in solution.
Typical Dosage for Dimenhydrinate
Adults: 50 - 100 mg 6 - 8 hours daily
For motion sickness drug should be administered half an hour before traveling.
Treatment of nausea and vomiting in Meniere`s disease: 50 mg IM for acute attack
Maintenance dose: 75 - 150 mg / day in 3 divided doses
Children: 1.5 mg / kg / day in 3 divided doses.

Store at 20 - 25 degree C in a tightly closed container. Keep out of the reach of children.
Effects of Missed Dosage of Dimenhydrinate
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.
Effects of Overdose of Dimenhydrinate
Give supportive measures and symptomatic treatment. Gastric lavage can be done to remove the drug from the body. To treat seizure Diazepam or Phenytoin is given to the patient.
