About Salmeterol
A long-acting beta2-adrenergic agonist, Phenethylamine derivative, Bronchodilator(Antiasthma).
Mechanism of Action of Salmeterol
Salmeterol is a long acting ?2 receptor agonist. It directly relaxes the airway smooth muscles and produces bronchodilation. Stimulation of ?2 receptors activates Gs adenylyl-cyclase -cyclic AMP path way and produces reduction in smooth muscle tone. ?2 receptor agonists also increases the conductance of large Ca2+ sensitive K+ channels in airway smooth muscles and leads to membrane hyperpolarisation and relaxation of smooth muscles. Its ?2 receptor selectivity is about 50 times greater than Salbutamol.
?2 receptor agonists also suppress the release of Leukotrienes and histamine from the mast cells in the lung tissue and enhance mucociliary function and decrease micro vascular permeability and also inhibits Phospholipase A2 which produces inhibition of prostaglandins production. Thus Salmeterol can inhibit the broncho-constriction produced by inflammatory mediators.

Typical Dosage for Salmeterol
Adult: Long term maintenance treatment of asthma, Prevention of Bronchospasm, Reversible obstructive airway disease: 2Inhalations of 25mcg 2 times daily in the morning and in the evening. Dose can be increased to 100mcg if needed.
Prevention of exercised induced Bronchospasm:2 Inhalations of 25mcg 0.5 - 1 hour before exercise.
Chronic obstructive pulmonary disease or emphysema: single oral inhalation of 42 - 60 mcg.
Children above 12 years: same as adult dose.

About Fluticasone
Synthetic topical glucocorticoid, Anti inflammatory and antipruritic in corticosteroid-responsive dermatoses; atopic dermatitis.
Mechanism of Action of Fluticasone
It reduces prostaglandin synthesis by inhibiting cyclo oxygenase enzymes both COX1 and COX2.In higher doses it reduces the thromboxane A2 synthesis by COX1 inhibition. So platelet aggregation is inhibited by this drug. After topical application in the case of ophthalmic preparation it prevents miosis by reducing ocular prostaglandins synthesis. It is a phenylalkalonic acid derivative Non Steroidal Anti Inflammatory Drug.
