Mechanism of Action of Penicillin G Benzathine
Penicillin G Benzathine is one of the members of beta lactum antibiotics. It is a bactericidal drug. It binds to the penicillin binding protein in the cell wall of bacteria and inhibits the enzyme transpeptidase which is responsible for the transpeptidation reaction in peptidoglycan synthesis. It also activates the endogenous enzyme which degrades the peptidoglycan.These results in inhibition of protein synthesis and there by cell wall synthesis. This is more active against gram positive organism than gram negative organism.

Interactions for Penicillin G Benzathine
Aminoglycosides, parenteral : Certain parenteral penicillins may inactivate certain aminoglycosides in vitro - do not mix in the same I.V. solution.
Anticoagulants : Large I.V. doses of penicillins prolong bleeding time - chances of bleeding.
Oral contraceptives : Efficacy of oral contraceptives reduced. Additional form of contraception during penicillin therapy is advisable.
Heparin : With parenteral penicillins there is increased risk of bleeding.
Erythromycin : Both synergism and antagonism have been reported.
Allopurinol : Rate of ampicillin-induced skin rash appears much higher when co-administered with allopurinol.
Tetracyclines : Bacteriostatic effects of tetracycline derivatives may impair bactericidal effects of penicillins.
Probenecid : Prolongs half-life of penicillin.

Typical Dosage for Penicillin G Benzathine
ADULT
12lac - 24lac units IM as a single dose
Primary and secondary syphilis: 24lac units once daily; Tertiary and late syphilis: 24 lac units once weekly for 3 weeks
Prophylaxis of rheumatic fever: 600000 units every 2 week
CHILDREN
General dose: 3lac-12lac units as a single dose
Congenital syphilis: 500000units/kg IM as single dose.
