About Mebendazole
A benzimidazole derivative, Anthelmintic.
Mechanism of Action of Mebendazole
Mebendazole is a benzimidazole used as a broad spectrum anthelminthic which acts by blocking glucose uptake in the parasite and depletion of its glycogen stores. Intracellular microtubules in the cells of the worm are gradually lost. The drug binds to beta tubulin of susceptible worms with high affinity and inhibits its polymerization. Ascaris ova are killed. It also inhibits hatching of nematode eggs and larvae.

Typical Dosage for Mebendazole
Adults & Children over 2 years:
Enterobius:100mg single dose, Repeats after 2 to 3 weeks
Round worm, Hook worm and Trichuris:100mg 12hourly for 3days; second course after 14 days if necessary
Tape worm: 0.2gm 12 hourly for 3 days
Trichinella spiralis: 200 to 400mg 12hourly or 8hourly for 3days
Hydatid disease: 200 to 400mg 12hourly or 8hourly for 3 to 4 weeks

Store in a well closed container at a temperature range of 15 to 25 degree Centigrade.
Effects of Missed Dosage of Mebendazole
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.
Effects of Overdose of Mebendazole
Treatment is supportive & symptomatic. Empty stomach by induced emesis & gastric lavage; within 4 hours after ingestion. Give activated charcoal to reduce absorption.

Mechanism of Action of Levamisole
Levamisole is the levo isomer of Tetramisole which is used as an anthelminthic agent. It exerts its action by producing paralysis of worm by causing sustained contraction of somatic muscles of worm by an irreversible, noncompetitive depolarization type of Neuro muscular block. It also non competitively inhibits the production of succinate.Its immunostimalant action is by modulating cell mediated immunity by increasing phagocytosis by macrophages. It restores cutaneous delayed hyper sensitivity reactions.

Alcohol: Levamisole may produce disulfiram like effects with alcohol.
Phenytoin: Co-administration with levamisole and fluorouracil results in increased serum phenytoin levels.
Typical Dosage for Levamisole
Adults:
Ascariasis:2.5 to 5mg/kg or 150mg single oral dose.
Ancylostomiasis & Mixed Ascaris, Hookworm infestation: 0.3gm for 1 to 2 days. Alternatively administer 2.5mg/kg as a single dose. Repeat after 1week if required
Children:
Ascariasis:3mg/kg or 50mg single oral dose.
Ancylostomiasis & Mixed Ascaris, Hookworm infestation: 6mg/kg for 1 to 2 days
