Mechanism of Action of Lanthanum Carbonate
Fosrenol is a carbonate salt of the rare-earth metal lanthanum. This salt dissociates in the acid of the stomach to release lanthanum ions that bind dietary phosphate released from food during digestion. Binding phosphate ions forms highly insoluble lanthanum-phosphate complexes, which are not absorbed in the rest of the GI tract. Dietary phosphate thus cannot contribute to serum phosphate levels, allowing the kidneys to reduce these levels to normal reanges. Lower serum phosphate levels also reduce the prodution of calcium phosphate, high levels of which can lead to ectopic calcification.

Typical Dosage for Lanthanum Carbonate
Adult: Initially, 0.75-2.25 g daily, given in divided doses with meals. Dose may be adjusted every 2-3 wk until an acceptable serum phosphate concentration is achieved; usual maintenance dose: 1.5-3 g daily in divided doses. Max: 3.75 g daily.
Should be taken with food. (Take w/ or immediately after meals. Chew thoroughly before swallowing. Do not swallow whole.)

The symptoms associated with overdose are adverse reactions such as headache, nausea and vomiting. In clinical trials in healthy adults, GI symptoms were reported with daily doses up to 6000 mg/day of lanthanum carbonate administered with food. Given the topical activity of lanthanum in the gut, and the excretion in feces of the majority of the dose, supportive therapy is recommended for overdosage.
