Mechanism of Action of Trastuzumab
The HER2 (or c-erbB2) proto-oncogene encodes a transmembrane receptor protein of 185 kDa, which is structurally related to the epidermal growth factor receptor. Trastuzumab has been shown, in both in vitro assays and in animals, to inhibit the proliferation of human tumor cells that overexpress HER2.Trastuzumab is a mediator of antibody-dependent cellular cytotoxicity (ADCC). In vitro, Trastuzumab mediated ADCC has been shown to be preferentially exerted on HER2 overexpressing cancer cells compared with cancer cells that do not overexpress HER2.

Indications for Trastuzumab
For treatment of early stage HER2-positive breast cancer, or metastatic breast cancer that substantially overexpress HER2.
Interactions for Trastuzumab
1.Increased risk of severe cardiotoxicity with anthracyclines.
2.Increased risk of leukopenia and anaemia in combination chemotherapy (e.g. paclitaxel).
3.Paclitaxel may decrease the clearance of trastuzumab.
4.Increased risk of bleeding with warfarin.
Typical Dosage for Trastuzumab
Trastuzumab usually is administered intravenously with an initial dose of 4 mg per kilogram of body weight followed by a weekly dose of 2 mg per kilogram of body weight.
