Functionalized amino acid, Anticonvulsant.
Mechanism of Action of Lacosamide
Precise mechanism of action is unknown; however, lacosamide selectively enhances slow inactivation of voltage-gated sodium channels, resulting in stabilization of hyperexcitable neuronal membranes and inhibition of repetitive neuronal firing.
Lacosamide does not affect AMPA, kainate, NMDA, GABAA, GABAB or a variety of dopaminergic, serotonergic, adrenergic, muscarinic or cannabinoid receptors and does not block potassium or calcium currents.
Pharmacokinets of Lacosamide
Absorption:Well absorbed orally.
Distribution:Vd is approximately 0.6 L/kg. Less than 15% is bound to plasma proteins.
Metabolism:Primarily cleared from the systemic circulation by renal excretion and biotransformation. Lacosamide is a CYP2C19 substrate.
Elimination:Approximately 95% is eliminated in the urine and less.

Contra-indications of Lacosamide
Lacosamide is contraindicated with patients with known second- or third-degree atrioventricular block. Prolongations in the PR interval have been observed in clinical studies.

1.Lacosamide is indicated as adjunctive therapy in the treatment of partial-onset seizures with or without secondary generalisation in patients with epilepsy aged 16 years and older.
2.Diabetic neuropathic pain

Typical Dosage for Lacosamide
Oral lacosamide should be initiated at 50 mg twice daily, increasing to 100 mg twice daily after one
week. The dose may be incrementally increased up to 200 mg twice daily. Discontinuation should
be performed gradually.
