Mechanism of Action of Mecobalamin
Mecobalamin is the neurologically active form of vitamin B12 and occurs as a water-soluble vitamin in the body. It is a cofactor in the enzyme methionine synthase, which functions to transfer methyl groups for the regeneration of methionine from homocysteine. In anaemia, it increases erythrocyte production by promoting nucleic acid synthesis in the bone marrow and by promoting maturation and division of erythrocytes.

Oral: Adult: 1500 mcg/day in 3 divided doses
Parenteral: 500 mcg daily IM/IV 3 times/wk

Megaloblastic anaemia caused by vitamin B12 deficiency
Parenteral: Adult: 500 mcg daily IM/IV 3 times/wk. Maintenance dose: After about 2 mth of therapy, reduce dose to single admin of 500 mcg every 1-3 mth.

Mechanism of Action of Piracetam
Piracetam is a cyclic GABA derivative. It facilitates the movement of information between the brain`s 2 hemispheres via corpus callosum and improves the function of Acetylcholine via muscarinic receptors which are implicated in memory process. Interactions for Piracetam
Oral anticoagulants: Increased prothrombin time.
Typical Dosage for Piracetam
Adults: Ischemic stroke: 12gm I.V./20 minutes, followed by 12gm I.V./day for 4 weeks or orally.
Post-stroke complications (aphasia/hemiplegia): 4.8gm/day for 8 weeks.
Age associated problems (memory lapses, vertigo, concentration gap): 2.4-4.8 gm/day for 12 weeks
Learning difficulties: 3 gm/day in 3 divided doses for 1 school year.
Children: 50 mg/kg body weight in 3 separate doses. Once the desired result has been obtained reduce the initial dose by half upto 6-8 weeks.
