A semisynthetic Aminoglycosides, Protein Synthesis Inhibitor, Antibiotic.
Mechanism of Action of Netilmicin
Netilmicin exerts its bactericidal action against gram- negative organisms & some of gram positive organisms by inhibiting bacterial protein synthesis. The process involves 1. Penetration of bacterial cell membrane & binding directly to the bacterial ribosome, 2. Misleading of m-RNA codon by bacterial ribosome, 3. Formation of peptide chains with wrong amino acid sequence which gets incorporated into bacterial cell membrane and alters its permeability, 4. Bacterial cell lysis. Some bacterial strains resistant to other amino glycosides are sensitive to Netilmicin & it is less toxic than other amino glycosides

Absorption: Poorly absorbed orally, Distribution: Distributed extracellularly. CSF penetration & intraocular penetration poor. Metabolism: Not metabolized, Excretion: Excreted mainly through urine.

Indications for Netilmicin
1.Infections caused by sensitive organisms like klebsiella, enterobacter, staphylococci etc.
2.Urinary tract infections
3.Skin ulcers
4.In neutropenic patients
5.Respiratory infections
6.Burn infections
7.Infections caused by Gentamicin resistant strains
Interactions for Netilmicin
Aminoglycosides, vancomycin, methoxyflurane, enflurane, cephaloridine: Increased nephrotoxicity.
Loop diuretics: The auditory toxicity appears to be increased.
Polypeptide antibiotics: Concurrent use of these agents may increase risk of respiratory paralysis and renal dysfunction.
Anaesthetics, neuromuscular blocking agents (Tubocurarine, Gallamine): Risk of neuromuscular blockade and respiratory paralysis.
Pencillin, cephalosporins, carbenicillin, ticarcillin: Exert antibiotic synergism, especially useful in psuedomonas infections.
Lab tests: Aminoglycoside serum level: Guard against in vitro inactivation of aminoglycosides by beta-lactum antibiotics in patients on combination therapy.
Typical Dosage for Netilmicin
Adults: 4 to 6 mg/kg/day in 1 to 3 divided doses. In severe infections dose may be increased up to 7.5mg/kg/day in 3 divided doses.
Children: 5.5 to 8 mg/kg/day in 2 - 3 divided doses.
Neonates: 4 to 6.5 mg/kg/day in 2 divided doses.
Urinary tract infections: 1.5 to 2mg/day in 2 divided doses.

Store at 15 - 30 degree C in a tightly closed container. Protect away from heat and light. Keep out of the reach of children.
Effects of Missed Dosage of Netilmicin
Take the missed dose as soon as noticed and if it is the time to take the next dose then skip the missed dose
Effects of Overdose of Netilmicin
Give supportive measures and treatment. An overdose results in Ototoxicity, nephrotoxicity, neuromuscular disorders & hypersensitivity manifestations. Treatment includes removal of drug by dialysis, & drug treatment with neostigmine, Ca, & Prednisolone.
