Mechanism of Action of Sisomicin
Sisomicin exerts its bactericidal action against gram- negative organisms & some of gram positive organisms by inhibiting bacterial protein synthesis. The process involves 1. Penetration of bacterial cell membrane & binding directly to the bacterial ribosome, 2.Misleading of m-RNA codon by bacterial ribosome, 3.Formation of peptide chains with wrong amino acid sequence which gets incorporated into bacterial cell membrane and alters its permeability, 4.Bacterial cell lysis.

2. Bacterial conjunctivitis
3. Keratitis
4. Keratoconjunctivitis
5. Trachoma
6. Blepharitis
7. Acute meibomianitis
8. Dacrocystitis
9. Before and after ocular surgery
10. Ocular infections
Interactions for Sisomicin
Aminoglycosides, Amphotericin B, Bacitracin, Cisplatin, Cephalothin, Vancomycin, Methoxyflurane Ethacrynic Acid, Frusemide, Bumetanide & Mannitol: Increased ototoxic, neurotoxic or nephrotoxic affects.
Anaesthetics, Neuromuscular blocking agents (Tubocurarine, Gallamine): Risk of neuromuscular blockade and respiratory paralysis.
Penicillin, Cephalosporins, Carbenicillin, Ticarcillin: Exert antibiotic synergism, especially useful in pseudomonas infections.
Lab tests: Aminoglycoside serum levels: Guard against in-vitro inactivation of aminoglycosides by b-lactum antibiotics in patients on combination therapy.

Typical Dosage for Sisomicin
The usual dose for adults is 3 mg/kg daily given intramuscularly in 2 or 3 divided doses. It may be given by intravenous infusion if necessary.
Adult: 1 - 2 drop to be instilled in to the infected eye every 4-6 hours
Trachoma: Treatment is for 1 - 2 months

Store at cool dry place. Protect from light. Do not freeze. Keep out of the reach of children.
Effects of Missed Dosage of Sisomicin
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.
Effects of Overdose of Sisomicin
Give supportive measures and symptomatic treatment. The exposed eye should be irrigated with copious amounts of water for at least 15 minutes.
