Mechanism of Action of Dextrose 50%
Dextrose restores blood glucose levels in hypoglycemia and provides a source of carbohydrate calories after IV injection. It decreases losses of body protein and nitrogen, promotes glycogen deposition, and prevents ketosis. It reverses CNS effects of hypoglycemia by rapidly increasing serum glucose levels.
Dextrose injection undergoes oxidation to carbon dioxide and water. Water is an essential constituent of all body tissues Average normal adult requirement ranges from two to three liters. Water balance is maintained by various regulatory mechanisms. Water distribution depends primarily on the concentration of electrolytes in the body compartments.

Contra-indications of Dextrose 50%
1.Hypersensitivity to dextrose solution, corn or corn products
2.Diabetic coma while patient is hyperglycemic
3.Intracranial haemorrhage
4.In the presence of delirium in dehydrated patients.
Special Precautions while taking Dextrose 50%
1.Diabetes mellitus
2.Ischemic stroke
3.Hypervolemia
4.Renal impairment
5,Urinary tract obstruction
6.Essential electrolytes, minerals, and vitamins should be supplied if needed
7.Potassium should be added to dextrose solutions administered to fasting patients with good renal function, especially those on digitalis therapy
8.Do not administer D50W if there is any doubt about IV patency.

1.Severe hypoglycemia
2.As a source of carbohydrate calories
3.Severe hyperkalemia
4.Dialysis-induced muscle cramps in uraemic patients
5.To promote diuresis.

Interactions for Dextrose 50%

Typical Dosage for Dextrose 50%
ADULT
Severe hypoglycaemia: 20 - 50 ml, repeat if needed
Severe hyperkalemia with ECG changes: 50 ml, repeat in 1 hour if finger stick glucose is less than 100 mg/dl
Dialysis induced muscle cramps: 50 ml and repeat in 5 minutes if there is no response
Acute attacks of porphyria: 40 ml/hour. If no improvement within 2 days alternative therapy should be considered.
PAEDIATRIC
Children above 3 years
Severe hypoglycemia: 1 - 2 ml/kg/dose, repeat if needed.
Severe hyperkalemia with ECG changes: 1 ml /kg/dose
Children under 3 years: not recommended
