About Clindamycin
Macrolide, Lincosamide Antibiotic.
Mechanism of Action of Clindamycin
It binds to the 50S sub unit of bacterial ribosome and inhibits interfere with the transfer of the newly formed peptide chain from the A site to the P site and fails to expose the A site .So that A site is unable to bind with the next aminoacyl t RNA complex. This leads to premature termination of amino acid chain and there by inhibits protein synthesis. Along with Quinine sulfate, Clindamycin is used in the treatment of Chloroquine resistant falciparum malaria.

Absorption: It is well absorbed after oral administration.Distribution: It is very widely distributed in the body as protein bound form.Metabolism: Small amount is metabolized in the liver to inactive compound.Excretion: The drug is excreted through urine and small amount through bile and breast milk.

Interactions for Clindamycin
Neuromuscular blocking agents: Efficacy of these agents (e.g. tubocurarine) enhanced.
Cholinergics: Effects of neostigmine and pyridostigmine antagonised.
Erythromycin: Antagonism
Typical Dosage for Clindamycin
Adult: 150 - 300 mg four times daily
Severe infection: 300 - 450 mg four times daily
Malaria: 450 mg 3 times daily for 7 days in a regimen with quinine sulfate
Bacterial vaginosis: 300mg twice daily for 7 days
Children:8 - 16 mg / kg body weight/day in 3 - 4 divided dose
Severe infection: 16 - 20 mg/ kg body weight/ day in 3 - 4 divided dose

Effects of Missed Dosage of Clindamycin
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.
