A tricyclic glycopeptide antibiotic.
Mechanism of Action of Vancomycin
Vancomycin binds to the terminal dipeptide sequence of peptidoglycan and inhibits the association and cross linking of peptidoglycan and as the result inhibit cell wall synthesis.

Absorption: It is well absorbed after parenteral administration.
Distribution: It is very widely distributed in the body.
Excretion: It is excreted through urine.

Indications for Vancomycin
1. Endocarditis
2. Pseudomembranous enterocolitis
3. Methicillin resistant Staphylococcus aureus infection
4. Staphylococcal enterocolitis
Interactions for Vancomycin
Anion-exchange resins: Antagonism of oral vancomycin by cholestyramine.
Aminoglycosides: Risk of nephrotoxicity may be increased.
Anaesthetics: Erythema and histamine like flushing in children.
Nondepolorizing Muscle Relaxants: The neuromuscular blockade may be enhanced.

Typical Dosage for Vancomycin
Adult (IV dose):500 mg every four times a day
Enterocolitis (oral) 0.5 - 2 g / day in four divided dose for 10 days
Children :( IV dose) 40 mg /kg body weight / day in four divided dose
Neonates: Initial dose: 15mg / kg followed by 20 mg / kg body weight /day in two divided dose for 1 week and then in three divided dose up to 1 month.

Store in room temperature. Keep away from moisture and light.
Effects of Missed Dosage of Vancomycin
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.
Effects of Overdose of Vancomycin
Give supportive treatment and measures. Maintain glomerular filtration rate. Then if needed haemodialysis or haemoperfusion can be done.
