3rd generation beta blocker,cardio selective beta-1 antagonist with beta-2 selective agonist, Anti hypertensive, anti angina.
Mechanism of Action of Celiprolol
Celiprolol is a 3rd generation beta blocker; cardio selective beta-1 receptor antagonist with additional beta-2 selective agonistic action which reduces vascular resistance and exerts anti hypertensive effects. It has negative chronotropic effects and bronchodilatory actions The drug causes coronary vasodilatation and improves myocardial ischaemia through nitric oxide release. It does not harm lipid profile, doesn`t harm carbohydrate metabolism & doesn`t harm myocardium in left ventricular dysfunction. In left ventricular hypertrophy it lowers serum fibrinogen levels and cause regression of myocardial mass.

Indications for Celiprolol
1.Hypertension
Interactions for Celiprolol
Ace inhibitors: Enhanced hypotensive effect.
NSAIDs: Antagonise hypotensive effect.
Antidiabetics: Enhanced hypoglycaemic effect.
Clonidine: Increased risk of withdrawal hypertension.
Cardiac glycoides: Increased peripheral vasoconstriction.
Ergotamine: Increased peripheral vasoconstriction.
Sympathomimetics: Severe hypertension.
Typical Dosage for Celiprolol
200mg once daily. Increased by each 200mg up to 600mg at 2weeks intervals if required based on patient`s response.

Effects of Missed Dosage of Celiprolol
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.
Effects of Overdose of Celiprolol
Treatment is supportive and symptomatic. After ingestion empty the stomach by induced emesis or gastric lavage. Administer activated charcoal to reduce absorption.Treat Bradycardia with Atropine. If required administer Isoproterinol with caution; in heart block. Treat hypotension with glucagons and vasopressors like Epinephrine. Treat cardiac failure with diuretics and cardiac glycosides
