Selective calcium channel blocker, Dihydropyridine derivative, Antihypertensive.
Mechanism of Action of Lacidipine
Lacidipine is a vasoselective long acting dihydropyridine Ca channel blocker. It exerts antihypertensive action through blocking the influx of Ca ions through voltage gated L-type Ca channels to the peripheral vascular smooth muscle cells, Coronary smooth muscle cells and to the myocardial cells. Thus causes dilatation of vascular endothelium, decrease peripheral resistance, and blood pressure. It exerts some anti atherosclerotic properties by modulating the secretion of matrix degrading metalloproteinase-9 by human macrophages and thus weakens the atherosclerotic plaque. It also has antioxidant properties and it prevents platelet aggregation.

Absorption: Rapidly but poorly absorbed orally, bioavailability is reduced due to first pass metabolism.
Distribution: Widely distributed in a protein bound form.
Metabolism: Metabolized in liver.
Excretion: Excreted mainly through bile and faeces and small amount is excreted through urine.

1. Congestive heart failure
2. Cardiovascular insufficiency
3. Hypotension
4. Left ventricular outflow obstruction
5. Conduction abnormalities
6. Hepatic impairment
7. Slowly withdraw the drug with caution.
8. Use cautiously along with other antihypertensive drugs.
9. Stops the drug if ischaemic chest pain or Cardiogenic shock occurs after starting therapy.

Avoid grape fruit juice as it may affect metabolism. Other interactions as with calcium channel blockers.
Typical Dosage for Lacidipine
Starts with 2mg once daily. Increased to 4mg/day if required after 3 to 4 weeks
Maximum dose: 6mg

Effects of Missed Dosage of Lacidipine
Take the missed dose as soon as remember and If it is the time for next dose then skip the missed dose and take the regular dose.
Effects of Overdose of Lacidipine
Treatment is supportive and symptomatic.Monitor & support cardiac and respiratory functions. Measure the BP. Give cardiovascular support by elevating the limbs and maintaining fluid balance. Administer vasopressers like nor epinephrine and Calcium gluconate if required.
