About Streptokinase
A plasminogen activator, Antithrombotic.
Mechanism of Action of Streptokinase
It exerts it`s fibrinolytic action by activating natural fibrinolytic system. It causes lysis of thrombus or clot and thus recanalyses occluded blood vessels such as coronary artery. It combines with plasminogen to form an activator complex which causes proteolysis of plasminogen molecules to plasmin (fibrinolysin). Plasmin converts insoluble fibrin to soluble fibrin fragments.

Indications for Streptokinase
1.Acute myocardial infarction
2.Pulmonary embolism
3.Venous thrombosis
4.Arterial Thromboembolic disorders

Interactions for Streptokinase
Anticoagulants and antiplatelet agents: May cause bleeding complications. Therefore patients should be carefully monitored.
Lab tests: Marked decrease in plasminogen and fibrinogen. Increase in thrombin time (TT), activated partial thromboplastin time. (APTT), & prothrombin time (PT).
Typical Dosage for Streptokinase
Acute myocardial infarction: 1.5 million IU infused intravenously over one hour. 20000 IU as intracoronary loading dose; followed by 2000 IU/minutes for one hour intravenously as maintenance dosage.
Pulmonary embolism, venous thrombosis, and arterial Thromboembolism: Starts with 250,000 IU; I.V. over 30minutes. Followed by 100000 IU/hour I.V. for 3days for venous thrombosis and for one day for pulmonary embolism and 1to3 days for arterial Thromboembolism

Storage Requirements for Streptokinase
Store at controlled room temperature at a range of 15 to 30 degree C.
Effects of Missed Dosage of Streptokinase
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.
Effects of Overdose of Streptokinase
Discontinue the drug during over dosage and restart during stoppage of haemorrhage
