About Dipyridamole
Antiplatelet Agent(Platelet Aggregation Inhibitor), Vasodilator.
Mechanism of Action of Dipyridamole
The drug exerts it`s antithrombotic action by interfering with platelet aggregation. It inhibits phosphodiesterase, blocks uptake and degradation of adenosine (local mediator involved in auto regulation of coronary blood flow in response to ischaemia.), Potentiate prostacyclin (PG I2) and increase cyclic AMP in platelets and interferes with aggregation.
Pharmacokinets of Dipyridamole
Absorption: About 27% to 59% of drug is slowly absorbed orally.
Distribution: Widely distributed in a protein bound form and a small portion cross the placental barrier.
Metabolism: Metabolized in the liver.
Excretion: Excreted mainly through bile and small amount is excreted through urine.

3. It shows "coronary steal" phenomenon by dilating resistance vessels in non ischaemic zone as well and diverts already reduced blood flow away from ischaemic zone.
4. Recent myocardial infarction with haemodynamic instability
5. Sub valvular aortic stenosis

3. Inhibition of platelet adhesion in patients with prosthetic heart valves

Interactions for Dipyridamole
Oral anti-coagulants & anti-arrhythmic agent Adenosine: Efficacy enhanced by dipyridamole.
Antacids: Reduces efficacy of dipyridamole.
Aminophylline: Reverses vasodilatation effect of dipyridamole.
Aspirin: Useful combination for prevention of thromboembolic phenomenon.
Typical Dosage for Dipyridamole
Oral:
Thromboembolic disorders: Initially 100mg. gradually increases the dosage up to 450mg (in divided doses); as required by the patient.
Angina pectoris: 50mg 8th hourly to be taken 1 hour before meals.

Effects of Missed Dosage of Dipyridamole
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.

Give symptomatic treatment and maintain blood pressure.
