Mechanism of Action of Pravastatin Sodium

It is an active hydrophilic statin which competitively inhibits the conversion of 3-hydroxy-3-methyl glutaryl coenzyme A (HMG CoA) to mevalonate. The drug exerts it`s action by specific inhibition of enzyme; 3-hydroxy-3-methyl glutaryl coenzyme A reductase (HMG CoA reductase) and reduces cholesterol synthesis up to 25% at therapeutic doses. This results in compensatory increase in low density lipoprotein (LDL) receptor expression in liver which ultimately leads to increased receptor mediated uptake and catabolism of intermediate density lipoprotein(IDL) and low density lipoprotein (LDL). It causes lowering of LDL cholesterol and reduces hepatic synthesis of very low density lipoprotein (VLDL) and lowers plasma triglyceride levels. It also increases high density lipoprotein (HDL) levels. It has only little effects on cholesterol synthesis in other tissues It also decreases plasma fibrinogen levels.

Absorption: About 34% of drug is rapidly absorbed orally, and bioavailability is about 17%. Distribution: Distributed in to the liver by extensive first pass hepatic extraction and about 50% of drug is plasma protein bound, Metabolism: Metabolized in liver in to both active and inactive metabolites, Excretion: Excreted mainly through bile and rest is excreted through urine.

Interactions for Pravastatin Sodium

Bile Acid Sequestrants (BAS): When pravastatin is administered 1hour before or 4 hours after BAS, there is no clinically significant decrease in bioavailability or therapeutic effect.
Gemfibrozil : Combination therapy generally not recommended.

Typical Dosage for Pravastatin Sodium

Oral:
Starts with 10mg to 20mg once daily; at bed time. Increased if required based on patients response.
Maximum dose: 40mg
