Mechanism of Action of Doxapram
Doxapram acts by promoting excitation of central neurons. It produces respiratory stimulation by an action on peripheral carotid and aortic body chemoreceptors. As the dose is increased Doxapram stimulates respiratory centers in the medulla and other parts of brain. The respiratory stimulant action is manifested by an increase in tidal volume with a slight increase in respiratory rate.

Absorption: Doxapram is well absorbed after IV / IM administration. Distribution: It is very widely distributed throughout the body. Metabolism: Doxapram is metabolised in the liver. Excretion: Drug is excreted mainly in faeces and small amount in urine also.

Indications for Doxapram
1. Post anaesthesia respiratory stimulation
2. Drug induced CNS depression
3. Acute respiratory failure
Interactions for Doxapram
Sympathomimetics or MAOIs: Additive pressor effects.
Halothane, Cyclopropane & Enflurane: Delay initiation of therapy for at least 10 minutes following discontinuation of anaesthetics.
Muscle Relaxants: May mask residual effects of muscle relaxants.
Theophylline: Agitation, muscle fasciculation and hyperactivity.
Morphine: Antagonises morphine induced respiratory depression.
Alkaline solutions like Aminophylline, Frusemide & Thiopentone: Precipitation occurs.
Typical Dosage for Doxapram
Post anaesthesia respiratory stimulation as IV: 0.5 - 1 mg / kg body weight if necessary repeat after 1 hour
Drug induced CNS depression: Initial dose IV injection: 2 mg / kg body weight if necessary repeat after 5 minutes and again 1 - 2 hour until patient awakens.
Maximum daily dose: 3 g
Acute respiratory failure IV infusion: 1.5 - 4 mg / minute

Storage Requirements for Doxapram
Reconstituted solution is store at 15 - 30 degree C and it is stable for only 2 days. Do not freeze the solution. Keep out of the reach of children.
