Mechanism of Action of Sertraline
Sertraline blocks the re-uptake of neurotransmitter Serotonin from the synapse in to the presynaptic nerve terminal in the CNS. This will prolong the action of Serotonin (5HT) and it is useful in the treatment of depression.

Absorption: Sertraline is well absorbed after oral administration.
Distribution: It is distributed in the body in protein bound form.
Metabolism: It undergoes hepatic metabolism and produces active metabolites.

1. Hepatic impairment
2. Renal impairment
3. Epilepsy
4. Hypomania
5. Metabolic disorder
6. Alcoholics
7. Patient on MAOI therapy (Discontinue the MAO inhibitor at least 2 weeks before Sertraline is started)
8. Use cautiously in patient at risk for suicide
9. Use caution while driving vehicle, operating machine and activities requiring mental alertness or judgment

Interactions for Sertraline
Desipramine, diazepam, digoxin and alcohol : No significant interaction reported.
Lithium: On concomitant use, increased incidence of mild to moderate tremors has been observed.
Warfarin : Sertraline with warfarin may increase prothrombin time.
Typical Dosage for Sertraline
Adult
Panic disorder, Post traumatic stress disorder: Initial dose: 25 mg / day dose can be gradually increased every week if sufficient response does not occurs.
Maximum dose: 200 mg / day
Depression, Obsessive compulsive disorder: Initial dose: 50 mg / day dose can be gradually increased every week if sufficient response does not occur.
Maximum dose: 200 mg / day

Storage Requirements for Sertraline
Store at 15 - 30 degree C in a tightly closed container. Protect from direct light and heat. Keep out of the reach of children.
Effects of Missed Dosage of Sertraline
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose.
Effects of Overdose of Sertraline
Give supportive measures and symptomatic treatment. Activated charcoal can be given to reduce the absorption of the drug.
