About Paroxetine
A selective serotonin reuptake inhibitor (SSRI), Anti-depressant.
Mechanism of Action of Paroxetine
Paroxetine blocks the re-uptake of neurotransmitter Serotonin from the synapse in to the presynaptic nerve terminal in the CNS. This will prolong the action of Serotonin (5HT) and it is useful in the treatment of depression.

Pharmacokinets of Paroxetine
Absorption: Paroxetine is well absorbed after oral administration. Distribution: It is distributed through out the body in protein bound form. Metabolism: It undergoes hepatic metabolism and the metabolites are not as effective as Paroxetine. Excretion: Excreted mainly through urine and also through faeces.

1. Hypersensitivity to Paroxetine,

1.Hepatic impairment
2.Renal impairment
3.Myocardial infarction
4.Cardiac failure
5.Epilepsy
6.Hypomania
7.Alcoholics
8.Patient on MAOI therapy (Discontinue the MAO inhibitor at least 2 weeks before Paroxetine is started)
9.Use cautiously in patient at risk for suicide
10.Use caution while driving vehicle, operating machine and activities requiring mental alertness or judgment

1. Depression
2. Obsessive compulsive disorder
3. Panic disorder
4. Social anxiety disorder
Interactions for Paroxetine
Monoamine oxidase inhibitors: Avoid concomitant use since serious, sometimes fatal reactions have been reported. Paroxetine hydrochloride should not be used in combination with a MAOI, or within 14 days of discontinuing treatment with MAOI.
Warfarin: Concomitant administration should be undertaken with caution.
Drugs affecting hepatic metabolism: The metabolism and pharmacokinetics of paroxetine may be affected by the induction or inhibition of drug metabolizing enzymes.
Tricyclic antidepressants (TCA): Paroxetine hydrochloride may inhibit TCA metabolism.
Alcohol: Avoid alcohol while taking paroxetine hydrochloride.

Typical Dosage for Paroxetine
Adult
Depression:
Initial dose: 10 mg / day dose can be increased by 10 mg every 1 - 2 week intervals if sufficient response does not occur.
Maximum dose: 50 mg / day
Obsessive compulsive disorder, Panic disorder: Initial dose: 10 mg / day, dose can be increased by 10 mg every 1 - 2 week intervals if sufficient response does not occur.
Maximum dose: 60 mg / day
Panic disorder: 20 mg / day

Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose.

Give supportive measures and symptomatic treatment. Activated charcoal can be given to reduce the absorption of the drug.
