About Dopamine
Adrenergic Agonist Agent, Phenethylamine derivative, Inotropic agents,Neurotransmitter,Vasopressor.
Mechanism of Action of Dopamine
The drug exerts it`s pharmacological action by acting as a dopaminergic (both D1&D2) receptor and adrenergic (alpha 1 & ?1) receptor agonist. It dilates renal and mesenteric blood vessels through its D1 receptor action by increasing intracellular cyclic AMP. It increases glomerular filtration rate and Na+ excretion. It is a positive inotropic drug and increases cardiac out put and systolic BP. It also causes coronary vasodilatation. High doses causes increased peripheral resistance and renal vasoconstriction

It administered intravenously.
Distribution: Widely distributed throughout the body but does not crosses blood brain barrier.
Metabolism: Metabolized in to inactive metabolites in liver, kidney, plasma. About 25% is metabolized in to nor- epinephrine.
Excretion: Excreted mainly through urine as metabolites.

1. Occlusive vascular diseases
2. Cold injuries
3. Diabetic endarteritis
4. Along with MAO inhibitors
5. Gradually withdraw the drug with caution
6. Hypovolemia should be corrected before therapy using suitable volume expanders
7. Do not mix the drug with sodium bicarbonate or other drugs

1. Acute myocardial infarction
2. Cardiogenic Shock
3. Septic Shock

Typical Dosage for Dopamine
In shock to increase cardiac out put, blood pressure, and urine volume:
2 to 50mcg/kg/minute as I.V. infusion. Dosage adjustment is based on patient`s response. Rate may be increased by 1 to 4mcg/kg/minute at 10 to 30minute intervals until optimum response is achieved.
Short term management of heart failure:
Starts with 0.5 to 2mcg/kg/minute as I.V. infusion. Dosage may be increase up to 3mcg/kg/minute based on patient`s response.
Children: 2 to 20 mcg/kg/minute based on patient`s response.

Storage Requirements for Dopamine
Store at controlled room temperature at a range of 15 to 30 degree C.
Effects of Missed Dosage of Dopamine
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.
Effects of Overdose of Dopamine
Treatment includes dosage reduction or withdrawal of the drug. If that fails to lower blood pressure use a short acting alpha blocking agent.
