Mechanism of Action of Lamotrigine
Lamotrigine prolongs the inactivated state of voltage sensitive neuronal sodium ion channel either by increasing the efflux or decreasing the influx of sodium ion across the cell membrane and inhibits the high frequency firing of neurons.
It blocks the voltage sensitive sodium ion channel and stabilizes the presynaptic membrane. It also prevents the release of excitatory neurotransmitters such as glutamate and aspartate receptors

Absorption: Lamotrigine is well absorbed after oral administration. Distribution: It is distributed both in protein bound form (55%) and unbound form. Metabolism: it is extensively metabolised in the liver. Excretion: It is excreted mainly in urine and only a small amount in faeces.

Indications for Lamotrigine
1.Anticonvulsant (In the treatment of partial seizure, Generalised tonic-clonic seizure)
2.Lennox Gastout syndrome
Interactions for Lamotrigine
Phenytoin, carbamazepine, phenobarbitone and primidone: Enhance the metabolism of lamotrigine and may increase dose requirements.
Sodium Valproate: Reduces the metabolism of lamotrigine.
There is no evidence that lamotrigine causes clinically significant induction or inhibition of hepatic oxidative drug-metabolizing enzymes.
Typical Dosage for Lamotrigine
Adults: Initial dose: 50 mg/day for 2 weeks increase the dose to 100 mg /day in 2 divided doses for next 2weeks. Thereafter increase the dose by 100 mg every week until sufficient response is obtained.
Maintenance dose: 300 - 500 mg / day in 2 divided doses.
Patient on Valproic acid therapy: 25 mg every other day for 2 weeks followed by 25 mg daily for 2weeks. Thereafter increase 25 - 50 mg every 1-2 weeks.
Maintenance dose: 100 - 400 mg / day in 1- 2 divided doses

Store at 15 - 30 degree C. Protect from heat and moisture.
Effects of Missed Dosage of Lamotrigine
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose
Effects of Overdose of Lamotrigine
Give supportive measures and symptomatic treatment. Drug can be removed from the body by gastric lavage or by inducing emesis.
