A synthetic form of vitamin K (Vit. K1), A fat-soluble vitamin,antihaemorrhagic.
Mechanism of Action of Phytomenadione
Phytomenadione is a synthetic form of vitamin K.
Vitamin K acts as a cofactor in the synthesis of coagulation proteins; prothrombin, factors VII, IX, and X by liver. Vitamin K dependent changes in the final stage of synthesis of these coagulation proteins is gama-carboxylation of glutamate residues of these zymogen proteins and their descarboxy-forms converted in to active forms; which confers on them the capacity to bind Ca2+ and to get bound to the phospholipid surfaces which are essential properties for participation in the coagulation cascade.

1.Hypoprothrombinaemia caused by anticoagulants
2.For haemorrhagic disease in new born

Interactions for Phytomenadione
Anticoagulants : It may be necessary to increase the anticoagulant dose.
Mineral oil : May decrease Gl absorption of vitamin K.
Typical Dosage for Phytomenadione
Hypoprothrombinaemia caused by anticoagulants:
10 to 20mg as slow I.V. infusion over 3 to 5minutes or as I.V. infusion over 20 to 30minutes following withdrawal of anticoagulant. Monitor prothrombin levels after 3 hours and repeat the dosage if required.
Maximum daily dose: 40mg
For haemorrhagic disease in new born:
1mg I.M. Repeat if required three times daily. Up to 10mg; if required over 3months based on therapeutic response
Prophylaxis: 1mg I.M.
