Beta1 Selective Adrenergic Blocking Agent, Class II Antiarrhythmic Agent, Antiarrhythmic Agent.
Mechanism of Action of Esmolol
Esmolol is an ultra short acting cardio selective beta-1 adrenergic antagonist. It has negative chronotropic effects on heart. It decreases oxygen consumption; cardiac work and aortic pressure. It decreases nor adrenaline and renin releases. It decreases central sympathetic out flow. In sympathetic over activity, it prolongs systole by retarding conduction. The drug decreases BP in hypertensive individuals; both systolic and diastolic BP is reduced. It exerts class-2 anti arrhythmic properties and suppress adrenergically mediated ectopic activity.

Absorption: Rapidly absorbed after I.V. infusion.
Distribution: Rapidly distributed throughout the plasma.
Metabolism: Hydrolyzed rapidly by plasma esterases.
Excretion: Excreted mainly through urine as metabolites.

Typical Dosage for Esmolol
Supra ventricular tachycardia: Starts with a loading dose of 500mcg/kg/minute for 1 minute. Followed by a 4-minute maintenance infusion of 50mcg/kg/minute. If tachycardia is not subsides repeat the loading dose and maintenance infusion increased to 100mcg/kg/minute. Continue titration as above as repeating loading dose and with increase in maintenance dose with increments of 50mcg/kg/minute. After therapeutic goal is achieved omit the loading dose and reduce increase in maintenance dosage from 50mcg to 25mcg and increase the interval between titration from 5 to 10 minutes.
Maintenance dosage: 50 to 300mcg/minute
For post operative and intra operative tachycardia and hypertension: 1mg/kg I.V. bolus over 30 seconds followed by 150mcg/kg/minute. Then if required give a loading dose of 500mcg and done the same procedure explained above in Supra ventricular tachycardia

Storage Requirements for Esmolol
Store at controlled room temperature at a range of 15 to 25 degree C.
Effects of Missed Dosage of Esmolol
Take the missed dose as soon as noticed and if it is the time for next dose then skip the missed dose. Continue the regular schedule. Do not double the dose.
Effects of Overdose of Esmolol
Stop the drug immediately. Then treatment is supportive and symptomatic. Glucagon is effective to overcome the cardiovascular effects. An I.V. dose of 2 to 3mg is administered over 30 seconds and repeat if necessary, followed by infusion of 5mg/hour until patients condition has stabilized.
